Patriot Act Forms for Calyx Point® The US Patriot Act requires institutions to gather certain personal information about applicants before opening an account. They are also required to verify identity and complete an affidavit. Many believe that the new 1003 satisfies this requirement; however, there is no provision for the affidavit. An additional form is necessary for the person accepting the application to verify that they have reviewed the original identification documents presented by the borrower. The purpose is to decrease the changes of money laundering activities that could support terrorists. To comply, each institution must review applicants' identification from certain allowed sources.
